60 minute visit - complete assessment to determine root cause of injury, chiropractic adjustment, manual therapy, therapeutic exercise programming.

Follow Up Visit - $190 / $95
60 minute visit or 30 minute visit - reassessment, chiropractic adjustment, manual therapy, exercise programming.

Visit length dependent on the evaluation, the extent of your injury, and the recommended treatment plan.

Package of visits may be available dependent on your recommended treatment plan.

Consultation call - complimentary
20 minute phone call to discuss your needs, answer questions, and determine if my care is the right fit for you.

insurance

In order to provide the most complete, personalized service, I am out of network with all insurance providers.

I can provide you with a superbill that you can submit to your insurance provider. This is a special form that you can submit to try and receive reimbursement for health services rendered. You may also submit a superbill for HSA/FSA use. I do not provide assistance in this process.

WORK or auto accident? 

Unexpected work injuries and car accidents are stressful. It is important with these acute injuries that you obtain early intervention and consistent care.

Unfortunately, I am not accepting patients for worker’s compensation injuries nor personal injury liens at this time.
